Not medical advice or a diagnostic tool

MediSymp organizes information you provide. It does not diagnose, rule out a condition, prescribe treatment or tell you whether a medicine is safe for you. Its charts, patterns, reflections and briefs are for personal recording and discussion with a qualified healthcare professional. Using MediSymp does not create a clinician–patient relationship.

Always consult your doctor

Discuss symptoms, concerns and any health decisions with your doctor or another qualified healthcare professional. Do not start, stop or change a medicine, dose or treatment based on MediSymp. Check medicine questions with your doctor or pharmacist. Do not postpone care or disregard clinical advice because of anything shown in the app.

AI can get things wrong

Optional voice and text organization and reflections use artificial intelligence (AI). It can mishear words, miss context, add unsupported details or produce convincing but incorrect text. Medicine names, doses, dates, allergies and who a condition refers to need particular care. A clear-looking result is not a clinical verification.

Review proposed fields against your own words and records before saving or sharing. Correct mistakes, remove unsupported details, and leave uncertain information unconfirmed. Check medicines against your prescription or label. MediSymp does not have a clinician check your entries or AI output.

Patterns are observations

Your logs may be incomplete or inaccurate. A pattern does not prove what caused a symptom or whether a treatment worked. Missing entries do not mean you were symptom-free; an unchecked medicine does not prove a missed dose. Share observations with your doctor for interpretation alongside your clinical history.

Your doctor brief is your record

A brief presents the information you choose to include. It is patient-reported, may contain errors, and is not a verified medical record or clinical assessment. Review it and bring relevant original records to your appointment. Sharing a brief does not confirm that a doctor has received, reviewed or acted on it.

Not for emergencies or monitoring

No clinician monitors your journal. MediSymp does not contact emergency services or alert your doctor. Safety messages and reminders can be missed or unavailable; the absence of a warning does not mean you are safe. If you think you have a medical emergency, contact your local emergency services immediately. Do not wait for the app or an AI response.

Your information and choices

Online features process only the information described before you choose to send it. Manual check-ins are available without online processing. Read our privacy policy for storage, sharing, retention and deletion, and our subscription details for access and cancellation.
